Our Zero Tolerance Policy
Any patient displaying physical abuse, verbal abuse or any threatening behaviour, risks removal from the practice. Registering with us is acceptance of this policy.
Suggestions and Complaints
We hope that our patients will assist us in the smooth running of the practice and we are endeavouring to give you the best possible care and attention. Should you have any suggestions or complaints, please do not hesitate to bring these to the attention of Katrina Collins, who will inform the practice team accordingly.
The practice has a complaints procedure to follow, therefore should you feel you have cause to complain, please contact Katrina Collins.
At certain times our staff are under extreme pressure and if this is the case we hope that you will understand if any appointments are inadvertently delayed.
Complaining On Behalf of Someone Else
You can complain on behalf of a child for whom you have parental responsibility. You can complain to us on behalf of someone else who used our services. However, before we can discuss any aspect of their care with you, or start to investigate, we will first need to get written consent from the person themselves.
Obtaining Consent
We will contact the person who received the care from our service and request that they sign a consent form for you to complain on their behalf and for us to investigate the incident.
If the person is unable to give their consent because they do not have the mental capacity to make an informed decision at the time, consent will be requested from their lasting power of attorney, that is the person appointed by them to act in their best interests and make decisions on their behalf.
If you are complaining on behalf of a child for whom you do not have parental responsibility, consent will be requested from whoever holds parental responsibility for the child.
Patient confidentiality is respected at all times. We will not disclose to anyone any patient care information relating to a complaint without the appropriate consent.
To submit a complaint please read the process and then click the link below to complete and submit the form.
Summary Care Record
Your Summary Care Record will contain important information about any medicines you are taking, allergies you suffer from and any bad reactions to medicines that you have had.
Giving healthcare staff access to this information can prevent mistakes being made when caring for you in an emergency or when your GP practice is closed.
Your Summary Care Record will also include your name, address, date of birth and your unique NHS Number to help identify you correctly.
You may want to add other details about your care to your Summary Care Record. This will only happen if you ask for the information to be included. You should discuss your wishes with the healthcare staff treating you.

You can choose not to have a Summary Care Record. You need to let your GP practice know by filling in and returning an opt-out form.
