Dear Patient

Re: Your views on Merger of Derwent Crescent Medical Practice to Wentworth Medical Practice.

We are writing to you as a patient registered with the practice to inform you of the Partnership change and Merger of our GP surgery at 20 Derwent Crescent London N20 OQQ with Wentworth Medical Practice at 38 Wentworth Avenue London N3 1YL with effect from December 2022. Derwent Medical Practice has been located at its present site for many years and will remain so for the foreseeable future. Dr Lubin will step down as GP Partner and NHS work and his name will no longer appear on the GP Contract. Dr Lefley will also step down as a GP Partner however will continue as a salaried doctor and will remain seeing patients within Wentworth Medical Practice.

There will be no immediate changes to where services will be delivered after the partnership change in July 2022 and Merger in December 2022. To improve access for patients the partners have been exploring with NCL Integrated Care Board (ICB) formally NCL CCG whether there is an opportunity to offer all patients services from one premises at the Wentworth Medical Practice. The practice and NCL ICB will inform and write to patients later in the year providing more detail on the premises options and seek your views to ensure any change to where services are provided to meet your needs.

Frequently Asked Questions:

1.Why is Derwent Medical Practice considering a Merger with another practice?

Dr Jonathan Lubin the long-term GP Partner is retiring from NHS services in June 2022 and his joint GP Partner Dr Laura Lefley is stepping down from her current position. The Partnership change in July 2022 together with the merger in October 2022 with Wentworth Medical Practice will enable a larger clinical team and supporting teams to enhance the existing services available from both practices.

2. What changes will I notice as a patient?

All existing services will continue, and you may notice new faces amongst the clinicians and reception team as we begin to share staff across the two practices.

There are plans being explored with NCL Integrated Care Board (ICB) to assess whether it is feasible for the Derwent Medical Practice patient list to relocate to an improved purpose-built building at Wentworth Medical Practice at 38 Wentworth Avenue London N3 1YL. This remains in its early stages of planning and approval to relocate has not occurred yet.

We understand though that this proposal might affect some people more than others therefore before any final decisions are made, we would like to hear your views, recommendations, and any potential challenges this relocation might mean for you.

3. What does this mean for patients?

You do not need to do anything as you will continue to be registered with Derwent Crescent Medical Practice and the name of the surgery will remain the same. We confirm that no patients will be asked to leave because of the partnership change or merger. All staff both clinical and administrative will continue to work at the practice and there will be no change to our practice opening hours. The surgery telephone number will remain the same.

4. How can I share my thoughts?

There will be a deadline to complete and return your survey. If you need any help to complete this survey, you can call the surgery on 0208 446 0171 and a member of the team will go through it with you. Forms are also available within the surgery for you to complete.

5.What do I need to do next?

Nothing as you can continue to access your GP in the usual way.

The feedback we receive from you will form part of the NCL ICB application to merge the two surgeries and we will contact you later when we have further details

If you have any queries, comments, or concerns please feel free to contact the practice on the contact details below.

Yours sincerely,

Dr Jonathan Lubin and Dr Laura Lefley
